Buggy Use - Self Certification Form

The CGU has introduced self certification for buggy use by players in county competitions. This introduction falls in line with the England Golf Transportation Policy.

The relevant form should be submitted to the CGU as part of a request to use a golf buggy in a CGU competition. This form does not constitute proof of authorisation to use a golf buggy in competition. Successful applicants will receive advice from the CGU as proof of their authorisation to use a golf buggy in competition.

Please note that you cannot obtain permission to use a golf buggy for temporary injuries including broken bones, pulled or damaged muscles or ligaments, or any other injuries unless they have/will last more than 12 months.

A certificate shall be valid for the duration of the event specified in the advice.

Completion of the form indicates consent to the CGU processing and storing the personal data contained herein for the purpose of administering competitions and processing a request to use a golf buggy.

Please note that any expense incurred for completing this form and delivering it to the CGU is the responsibility of the Player.
